Beyond TSH: What a Comprehensive Thyroid Evaluation Can Tell You

Heather Bertolero

If you've ever been told that your thyroid labs are "normal" but you still struggle with fatigue, weight changes, brain fog, or low energy, you're not alone.

At Teleios, we believe thyroid health deserves a thoughtful, individualized evaluation. While Thyroid Stimulating Hormone (TSH) is an important screening test, it doesn't always tell the whole story.

Why Your Thyroid Matters

The thyroid gland produces hormones that influence nearly every organ system in your body. It plays a critical role in:

  • Metabolism
  • Energy production
  • Body temperature regulation
  • Heart function
  • Mood and cognitive performance
  • Weight management
  • Hair and skin health

When thyroid function is disrupted, symptoms can be subtle and easily mistaken for stress, aging, or hormonal changes.

Common Symptoms of an Underactive Thyroid (Hypothyroidism)

People with hypothyroidism may experience:

  • Persistent fatigue
  • Weight gain or difficulty losing weight
  • Cold intolerance
  • Constipation
  • Dry skin or brittle hair
  • Hair thinning
  • Depression or low mood
  • Brain fog or memory issues
  • Muscle aches or weakness

Common Symptoms of an Overactive Thyroid (Hyperthyroidism)

Symptoms may include:

  • Unexplained weight loss
  • Rapid heartbeat or palpitations
  • Heat intolerance
  • Anxiety or nervousness
  • Tremors
  • Frequent bowel movements
  • Excessive sweating
  • Sleep disturbances
  • Muscle weakness

Because these symptoms overlap with many other medical conditions, laboratory testing is an important part of the evaluation.

What Is Included in a Comprehensive Thyroid Assessment?

Depending on your symptoms and medical history, your provider may recommend several laboratory tests to better understand thyroid function.

TSH (Thyroid Stimulating Hormone)

TSH is often the starting point for thyroid evaluation. It reflects how the brain is signaling the thyroid gland and can help identify both underactive and overactive thyroid conditions.

Free T4

Free T4 measures the amount of circulating thyroxine available to your body and provides additional insight into thyroid hormone production.

Free T3

Free T3 represents the active thyroid hormone available to tissues and may be useful in selected clinical situations when interpreted alongside other laboratory values.

Thyroid Antibodies

Tests such as thyroid peroxidase (TPO) antibodies and thyroglobulin antibodies can help identify autoimmune thyroid diseases, including Hashimoto's thyroiditis and Graves' disease.

Additional Testing When Appropriate

In certain clinical scenarios, providers may order additional laboratory studies, including reverse T3 or other specialized testing. These tests are not necessary for every patient but may be considered based on individual circumstances and clinical judgment.

Thyroid Health and Weight

Many patients assume difficulty losing weight is caused by thyroid disease. While untreated hypothyroidism can contribute to weight gain, it is only one piece of a much larger picture.

Nutrition, physical activity, sleep quality, stress, medications, body composition, insulin resistance, and sex hormones all influence metabolism.

That's why Teleios takes a comprehensive approach rather than focusing on a single laboratory value.

Personalized Care Matters

Lab results should never be interpreted in isolation. The most effective treatment plans combine laboratory findings with your symptoms, medical history, physical examination, and overall health goals.

Our goal is to identify the root causes of your symptoms and develop a personalized strategy that supports long-term wellness and optimal performance.
